By checking below Foster Caregiver acknowledges they have read, understand and agree to all of the following:
1.

The Foster Caregiver acknowledges that he or she is not the owner of any foster animal but is willing to provide humane care for any such animal until its permanent placement in a suitable home.
2.

FLOD VT delivers to Foster Caregiver and Foster Caregiver hereby accepts from FLOD VT a certain animal or animals and Foster Caregiver agrees to humanely take care of said animal or animals until proper placement is found. When requested, Foster Caregiver will immediately, peaceably, and voluntarily deliver said animal or animals to FLOD VT and will make no claim of ownership, title, right, or interest in and to said animal or animals.

Foster Caregiver agrees to let FLOD VT inspect Foster Caregiver's premises where the animal or animals are being kept at any time to ascertain and satisfy itself that the animal or animals are well cared for. Foster Caregiver agrees to keep a collar and identification tag on any dog at all times.

Foster Caregiver agrees not to alter in any way the appearance of any animal being fostered without written permission of FLOD VT. This includes declawing and cropping of ears or tails.

Foster Caregiver agrees to provide adequate food, water, shelter, and kind treatment for all foster animals at all times and agrees to provide regular exercise for animals in your care. If you have a large dog and a small home, please plan to walk the dog on a leash daily. Keep dogs in a fenced yard when off-leash. They should only be allowed out of the yard on a leash and accompanied by you. They must always wear a collar with current identification. In addition, Foster Caregiver agrees to follow all written/verbal instructions from FLOD VT.

Foster Caregiver agrees to notify FLOD VT as to any behavioral or health problems of any foster animal. FLOD VT reserves the exclusive right to determine the proper course of action to take upon such notification. If FLOD VT determines that veterinary care is required, Foster Caregiver or a FLOD VT representative will transport the foster animal to a veterinarian specified by FLOD VT and at FLOD VT's discretion. The cost of any approved veterinary care will be paid by FLOD VT: (a) If medications are ordered by a veterinarian, FLOD VT will provide the medication. However, the foster is required to administer it. Some medications must be administered around the clock, or with specific foods, or under very specific circumstances. Instruction on medication administration will be provided to the foster. (b) Under no circumstances may the Foster Caregiver have the animal treated at his/her discretion or at a veterinarian not approved by FLOD VT. Any monies so expended will be the full responsibility of the Foster Caregiver.

Foster Caregiver will notify FLOD VT in the event any change occurs in the address or telephone number listed below. Foster Caregiver understands and acknowledges that he/she does not have any right or authority to keep any foster animal or place any foster animal in other homes or any place with other individuals unless permission is given in writing by FLOD VT.

Foster Caregiver is undertaking these obligations with no claim, now or in the future, to any type of compensation or reimbursement for caring for any foster animal, and the further consideration for undertaking this obligation and caring for said animals is that Foster Caregiver is receiving satisfaction and enjoyment from undertaking this obligation of his or her own free will and because he or she wants to do so and derives satisfaction from doing so.

Foster Caregiver agrees that accidental animal bites or other injuries to humans and other animals do occur and agrees to hold harmless and indemnify and protect FLOD VT from any claim or suit filed by anyone as a result of such an incident. In addition, FLOD VT will not be responsible if a foster animal should damage or destroy property belonging to Foster Caregiver or shall transfer any disease or internal or external parasites to other animals belonging to Foster Caregiver.

If the Foster Caregiver wishes to adopt said animal, Foster Caregiver must go through the FLOD VT adoption program screening process. FLOD VT reserves the right to determine final disposition of the animal.

This Agreement is the entire agreement of the parties, and there are no oral promises or representations made in addition to this contract, and it may only be changed in writing and signed by both FLOD VT.
Foster Caregiver's electronic signature below indicates that Foster Caregiver has read, understood and agrees to the conditions set forth in this agreement.

FOSTER CAREGIVER ATTESTS TO THE FACT SHE/HE HAS AUTHORITY TO ENTER INTO THIS AGREEMENT AND IS FREELY DOING SO. SHE/HE AGREES TO HOLD RESCUE, ITS FOUNDERS, DIRECTORS, OFFICERS, AGENTS, VOLUNTEERS, ESTATE, HEIRS AND ASSIGNS HARMLESS AND INDEMNIFY THEM FROM ALL LIABILITY, CLAIMS, JUDGMENTS OR EXPENSES OF ALL SORT OR ANY NATURE ARISING OUT OF OR IN CONNECTION WITH DOG.
