This agreement is between For the Love of Dogs Vermont and the above-named Foster-to-Adopt Parent (hereafter referred to as FTA Parent). For the Love of Dogs Vermont agrees to give the FTA Parent possession and control of Foster-to-Adopt Dog (hereafter referred to as Dog) contingent upon the terms outlined below. As a FTA Parent, you agree to the following terms: *

(By checking below) adopter acknowledges they have read, understand and agree to all of the following:

1. *

To treat the Dog as a fully adopted dog by providing food, shelter, and veterinary care and to assume all responsibility for the Dog’s well-being.

2. *

To ensure that the Dog lives primarily indoors and is NOT left outside unsupervised, even in a fenced yard, for extended periods. If the FTA Parent does not have a fenced yard, the Dog will live inside at all times and be taken outside on a leash on a regular basis for exercise and elimination needs.

3. *

To comply with all state and local laws relative to the keeping and care of the Dog.

4. *

To assume all responsibility for damage caused by the Dog, either directly or indirectly, while under the FTA Parent’s direct control.

5. *

To take every precaution necessary to ensure that the Dog is in reasonably good health. For the Love of Dogs Vermont cannot be held responsible/liable for any unforeseen health problem that may develop once the Dog is in the FTA Parent’s care. If unforeseen problems arise, the FTA Parent is responsible for all medical needs of their Dog.

6. *

To take the Dog to a vet of their choice for routine medical care, vaccinations, spay/neuter surgery, or any other medical treatments as deemed necessary, including fecals. All veterinary costs are the responsibility of the FTA Parent.

7. *

To indemnify and hold harmless For the Love of Dogs Vermont, and its Board of Directors, from any and all liability arising out of or in consequence of, or injury sustained as a result of, any activity connected with the Dog.

8. *

To be responsible for arranging for the safe care of the Dog in the event of travel and to agree to pay all related boarding expenses.

9.

To carry sufficient homeowner’s/renter’s insurance to cover injury or damage caused by the Dog and to be aware of any breed restrictions that may be in place.

10.

To contact For the Love of Dogs Vermont in the event that the FTA Parent decides they do not want to adopt and to await the finding of a new placement. The time frame for placement will depend on the availability of a foster who can take the Dog.

To sign an official adoption contract within 7 days after arrival for a puppy under 6 months, and within 14 days after arrival for puppies and dogs over the age of 6 months. In cases where medical or behavior needs are present, time can be extended as necessary. Our goal is to be sure ALL our animals get a forever home. Any length of time beyond the above must be communicated to and arranged with For the Love of Dogs Vermont.

12.

To pay the adoption fee up front for the Dog listed in this Foster-to-Adopt Contract. This fee is refundable (minus $150) if the dog is not adopted.
